The son of an oil wildcatter, Mr. Hazlewood was born in Mannford, Okla., on July 9, 1929.

After an itinerant childhood, he attended high school in Port Neches, Tex., served in the Korean War, attended broadcasting school and eventually found work as a D.J. in Arizona, where he met the guitarist Duane Eddy.
